The Weld County Diabetes Coalition is a community coalition  that was established in 1997. Some members provide self-management education to people with diabetes and other members work with populations that are at risk of developing diabetes. The coalition has helped train health care professionals about current diabetes treatments, purchased videos and books about diabetes for the High Plains Library District, provided scholarships for seniors for exercise classes, assisted with adult and juvenile diabetes support groups, and offered healthy cooking classes.
